In recent years, All-on-4 dental implants have been aggressively marketed as a fast, “permanent” solution for missing teeth. Many implant centers advertise this procedure with promises of same-day teeth and beautiful smiles.

I’m sharing my professional experience as an implant dentist in Encino CA, to help you understand why All-on-4 is not always the best choice—and why sometimes a little patience and planning can save you years of frustration, pain, and thousands of dollars.
1. Limited Number of Implants Creates a Weak Foundation in Encino CA
One of the main problems I see is that All-on-4 uses only four implants to hold an entire arch of teeth.
Of the four implants used in All-on-4, the two back implants are actually positioned closer to the middle of the arch. They are then angulated backward in an attempt to reach the molar area—but in many cases, they don’t truly engage the bone where your natural back teeth were.
This is not good for long-term stability. It creates a situation where the prosthesis ends up hanging off these implants, putting excessive leverage and stress on them.
In my experience, this is why those implants often overload and fail—it’s usually just a matter of time.
2. Looks Can Be Deceiving: A Pretty Smile Doesn’t Mean a Solid Foundation
A lot of All-on-4 advertising shows beautiful before-and-after photos.
But what you see is only the surface—the “look.”
I often compare it to a house that’s been freshly painted to look brand new. Underneath, the foundation may be unstable or crumbling.
In dentistry, the foundation is everything.
If implants aren’t properly positioned in the bone where your molars were, no amount of cosmetic finishing can prevent long-term problems.
3. One Size Fits All: Why Many Centers Push All-on-4
In my experience, one reason large implant centers promote All-on-4 is because it allows them to finish cases faster and avoid the extra steps of rebuilding bone or letting implants heal properly.
This approach is faster for the clinic, easier to schedule, and more profitable in the short term.
It’s also important to understand that there are many different arch shapes, and some are more favorable to All-on-4 than others. Unfortunately, in many implant centers, they don’t care and treat everybody the same, with total disregard for this factor—which can lead to premature failure.
4. Unnecessary Tooth Extractions: One Protocol, No Alternatives
One of the most concerning trends I see is that All-on-4 treatment almost always requires extracting all of your teeth—good and bad.
I have seen many cases where patients actually had healthy, savable teeth that could have been retained. But in All-on-4 clinics, there is only one cookie-cutter protocol: remove everything and place four implants.
There is no deviation and no discussion of alternative treatment plans that could combine implants and natural teeth.
In my practice, I believe your natural teeth should be preserved whenever possible.
5. The Illusion of “Cheaper” Treatment
All-on-4 is often presented as a less expensive option compared to full arch dental implants with more implants or bone grafting.
Yes, rebuilding bone and placing more implants adds to the initial treatment cost, and some patients feel worried that their dentist is “upselling.”
But here’s what I see over and over in my Encino practice:
When an All-on-4 fails, it can cost you the same—or sometimes double—to fix.
On average, All-on-4 treatments cost anywhere from $17,000 to $26,000 per arch, depending on materials, sedation, and other factors. If the prosthesis fails after a few years, you may face the same expense all over again—plus pain, discomfort, and going through surgery again.
And it’s heartbreaking to watch patients who once cried tears of joy when they first received their new teeth later cry in agony and frustration when the prosthesis fails—often at the worst possible time.
6. Don’t Be Misled About Longevity
Do not assume that when you get All-on-4, it lasts forever.
This is one of the most common misunderstandings I see.
It is the wrong message often relayed to patients, who are surprised and devastated when it fails. They thought that because they spent thousands of dollars and finally got their teeth, that would be the end of it.
Well, it is not.
These restorations require maintenance, repairs, and sometimes replacement—and when they fail, the consequences are serious.
7. Lack of Redundancy and Maintenance Challenges
When only four implants are used, there is no backup if one implant fails.
By contrast, when we use six or more implants placed correctly under the molars, the load is distributed, and you have a much stronger, more reliable foundation.
Additionally, All-on-4 prostheses are often made as a single-piece bridge. If it chips, fractures, or needs repair, the entire restoration must be removed, creating more appointments, higher costs, and major inconvenience.
These are critical issues most patients never hear about in the commercials—or at their consultation visit with those implant centers.
My Approach: Thoughtful, Individualized Treatment Planning

Place implants where they belong—under your molars, not just in the middle of the arch.
Build a foundation that respects your bone, your bite, and the forces of chewing.
For some patients, that means taking extra time to rebuild lost bone and allowing implants to integrate fully. It means preserving any healthy teeth you have instead of extracting them automatically. It means avoiding shortcuts and making sure the design is right for your unique situation.
While this approach sometimes costs a little more at the beginning, in my experience, it saves patients from much bigger problems later on.
Final Thoughts
Before you commit to any full arch implant treatment, be sure you are fully informed:
- Ask exactly how your implants will be positioned and why that specific plan was chosen for you.
- Discuss whether your remaining teeth can or should be saved instead of extracting everything by default.
- Get a second—or even a third—opinion before you let anyone remove all your teeth.
- Understand that the foundation is everything.
